STEP ONE - Prepare a team roster

You will need to upload a team roster with your order. You can create it in MS Word, Excel, or any text editor. Note where you save the file as you will upload it on the form below. You must include the following:

• Players - Include the players first name, last name or nick name (only one) and jersey number*
• Support Staff (Manager, Coaches, Team Parent(s) & Sponsor(s) - Include first and last names. Both names will be used whenever possible

All information must be submitted when placing this order.

*Jersey numbers can be submitted at the time of proofing, if they are unavailable when submitting the order

** Maximum file size = 1mb

Please review your order carefully before submitting.   You will be sent your banner proof for sign-off via email. Sign off is for spelling checks only, not redesign of your banner. Redesign of your banner will start the process over & could incur additional charges.
